Job Description: Consultant for Healthy Heart Africa Programme in Rwanda
Position Title: Consultant – Healthy Heart Africa Programme
Duration: 6 Months
Location: Gakenke, Gatsibo, and Nyarugenge Districts, Rwanda
Organization: PATH in Partnership with RBC

Project Overview:
The Healthy Heart Africa Programme aims to strengthen the capacity of primary healthcare facilities to deliver quality hypertension and diabetes services. The project is implemented in the health facilities of Gakenke, Gatsibo, and Nyarugenge districts. The focus is on enhancing primary healthcare delivery, facilitating community-based screenings, improving patient linkage to care and data use for decision-making.

Position Summary:
PATH is seeking a dedicated and dynamic consultant to support the implementation of the Healthy Heart Africa Programme. The consultant will be pivotal in coordinating community-based screening activities, monitoring and reporting data, and ensuring effective communication between stakeholders to achieve smooth implementation and wider impact of the programme.

Key Responsibilities:
1. Community-Based Screening Coordination:
•Plan, organize, and oversee community-based screening activities for hypertension and diabetes in the targeted districts.
•Collaborate with local health facilities and community leaders to ensure effective mobilization and participation in screening events.
•Ensure screening activities are conducted according to established protocols and guidelines.

2. Monitoring and Data Reporting:
•Collect, analyze, and report data on screening outcomes, patient follow-ups, and linkage to care.
•Provide regular updates and detailed reports to project stakeholders and funders.

3. Stakeholder Communication and Coordination:
•Facilitate effective communication between PATH, RBC, local health facilities, and other stakeholders.
•Organize and participate in regular meetings to discuss project progress, challenges, and solutions.
•Ensure timely dissemination of information and resources to all relevant parties.
4. Capacity Building and Support:
•Provide training and technical assistance to healthcare providers and community health workers involved in the programme.
•Assist in identifying and addressing barriers to effective hypertension and diabetes care in the target districts.

5. Reporting and Documentation:
•Maintain comprehensive records of all activities, meetings, and communications related to the project.
•Prepare detailed activities budgets and financial reports for accountability.
•Contribute to the development of strategic plans and recommendations for the sustainability and scalability of the programme.

•Bachelor’s degree in public health, Nursing, Social Sciences, or a related field. A master’s degree is preferred.
•Minimum of 5 years of experience in public health programme implementation, preferably in non-communicable diseases.
•Proven experience in coordinating community-based health interventions and screenings.
•Strong data management and analytical skills, with experience in monitoring and evaluation.
•Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
•Proficiency in English and Kinyarwanda; knowledge of French is an asset.
•Ability to travel frequently to the project sites within Gakenke, Gatsibo, and Nyarugenge districts.
